A 2 year old boy with an ultrarapid-metabolizer CYP2D6 phenotype who had undergone elective adenotonsillectomy was sent home with codeine. Two days later was found unresponsive with absent vital signs and a supratherapeutic morphine thought to be due to an increased conversion of codeine to morphine.
